Licensed Practical Nurse - The Licensed Practical Nurse is an associate nurse for residents in an assigned coverage area. TheLPN is responsible to direct the daily care of the resident by ensuring RTWs are following theprescribed plan of care and through effective communication to the covering Registered Nurse. TheLPN will assist the resident to achieve and maintain his/her optimal mental and physical functioningthrough direct care and delegation of appropriate tasks to the RTW.The LPN is responsible to assure plans of care are followed as prescribed or changes are implementedto ensure safety of the resident. The LPN communicates timely to the primary RN any changes madeto the care plan for safety purposes.
ABOUT THE JOB
The Licensed Practical Nurse (LPN) provides daily direct practical nursing care to residents and clinical leadership and directionto the RTWs. She/he complies with applicable standards of nursing care and established policies,procedures, and protocols. She/he attends report daily to direct staff assignments and workloaddistribution. She/he will participate in the daily care delivery at the bedside within the LPN scope ofpractice under general supervision of a registered nurse or physician.The LPN is a role model in all aspects of nursing practice. She/he identifies educational needs of theRTWs. The LPN follows the chain of command when utilizing resources for problems solving. She/he communicates areas of concern to RN and/or Nursing Supervisor in a timely manner. The LPN works in collaboration with the Registered Nurse and Nurse Clinician providing input intoassignments, ensuring best practices are followed in the delivery of care to meet the residents' needsand wishes.
RESPONSIBILITIES
- Provide daily direct practical nursing care to residents.
- Provide clinical leadership and direction to the RTWs.

- Participate in daily care delivery at the bedside within the LPN scope of practice under general supervision of a registered nurse or physician.
- Communicate areas of concern to RN and/or Nursing Supervisor in a timely manner.
QUALIFICATIONS
- Must be able to pass a background check.
- Must get a TB test.
ADDITIONAL ELIGIBILITY
Upon successful completion of SkillBridge program, offer them full-time, part-time or temp employment. The best case scenario is if they complete the SkillBridge program take employment with IVH and continue on to purse RN. IVH would like a commitment to take a full-time, part-time or temp position after completion of the SkillBridge program. Temporary transitional housing is available on the Iowa Veterans Home Campus.
