The Transitional Housing Program Support Specialist at Molding Hearts is responsible for providing direct support and structured services to individuals experiencing homelessness as they transition into stable housing and successful community reintegration. This role combines case management support, daily living observation, janitorial training, landscaping training, and life-skills development to help participants build independence, responsibility, and employable skills while acclimating back into society. The ideal candidate is compassionate, dependable, and committed to serving vulnerable populations with dignity, accountability, and encouragement.
The Transitional Housing Program Support Specialist internship is a comprehensive, 24-week vocational training program designed for transitioning service members looking to build a career in non-profit operations, program management, or supportive housing services. This multi-disciplinary role provides hands-on experience across three core pillars of transitional housing operations: Programmatic Case Management, Community Outreach, and Site Maintenance & Logistics. Service members will gain the cross-functional skills required to manage, maintain, and scale supportive housing communities for veterans. Some of the job specific duties will be:- Complete intake assessments, verify program eligibility, and input accurate client data into the Homeless.- Management Information System (HMIS).- Coordinate directly with the VA, healthcare providers, and local networks to connect veteran residents with benefits and medical care.- Document client progress and ensure all file management strictly aligns with compliance standards.- Conduct field outreach in the local community to identify, engage, and build trust with unhoused or at-risk persons.- Distribute programmatic resource materials and represent the organization at community action group meetings.- Facilitate intake orientations and program briefings for incoming service members and residents.- Perform routine facility safety inspections, log work orders, and maintain strict OSHA compliance across the property.- Manage inventory control for facility supplies, resident provisions, and operational equipment.- Execute basic site maintenance tasks, including minor physical repairs and native desert xeriscape upkeep.- Implement emergency management protocols and coordinate response workflows using FEMA Incident Command (ICS) frameworks during site incidents.
Service member are responsible for their own transportation.
